Physiotherapy Treatment of Joint Position Sense

The importance of proprioception has been covered in a previous article so this article concentrates on the assessment and management of problems due to loss of joint position sense. Physiotherapists concentrate mostly on pain, loss of joint ranges and poor muscle power initially as these are the commonest restrictions found clinically. However they will always consider joint position sense once the more typically troublesome symptoms are settling. Full rehabilitation of the patient back to normal or sporting activity cannot be said to be complete without retraining the proprioceptive function of the body’s joint and nervous systems.

Our arms are designed for one main function, to position our hands in a functional posture in front of our vision so we can see our activity and correct it. An effective and accurate feedback loop is necessary so we can assess the results of our efforts in real time and re-design our motor activities to get us closer to our goal. A major part of being human is being able to manipulate the world with our incredibly precise hands and detailed binocular vision. Understanding where our individual hand joints are at any one time is vital if we are going to use them effectively. Touch typing, which I am doing now, is a skill for which I must be certain where my hands are and where my fingers are going next.

Upper limb proprioception can be tested by the physiotherapist getting the patient to shut their eyes and then by putting their good arm into a specific position. Once the position has been set by the physiotherapist the patient is asked to put their other, affected, arm into the same position as closely as possible. A person whose joint position sense is unaffected can mirror position of one arm by using the other, very accurately. Any deficits in the ability to understand the position of the arm joints will be apparent after this test.

Proprioception in the legs and lower body is adapted for a different use than in the arms, lower body function being concentrated on bearing weight and walking. The disability from loss of proprioception in the legs can be very damaging as it affects independent mobility and balance. A good illustration of this problem is demonstrated by a multiple sclerosis patient I used to treat who had problems when it was dark.

My patient told me a short story with which many of us can identify. He went into a toilet and shut the door, locking it behind him. He automatically turned on the light, not noticing that the light was already on, something we have all probably done. Unfortunately for my patient, when the light went out so did the visual clues he was getting from his eyes gauging his movement in space, causing him to fall over at once! Since his proprioception was inaccurate he could not figure out where his legs where moment to moment and could not readjust any inaccuracies.

The physiotherapist might check the accuracy of the patient’s proprioception by asking them to place the leg in a position which they set, as in the arm test above. But more practical for the leg is to test the joint position sense in weight bearing and functional positions. The physiotherapist will observe the patient’s gait and this might be obviously abnormal, at least partly due to proprioceptive loss. If the loss is less obvious then the physiotherapist will test their balance with their feet placed together, moving on to one leg standing and even to standing on one leg with the eyes shut.

Proprioceptive ability can be re-trained by the physiotherapist who will initially ask patients to move their limbs under visual control, gradually withdrawing this until they can rely on the information pouring in from the bodily structures for co-ordinated movement. Adding compression by encouraging weight bearing through the joint can increase the proprioceptive input through the structures and improve the accuracy of input and of the movement response. Even though the patient may not be able to entirely regain their abilities, by working at the deficiencies they can continue to improve.

When you have the motivation to stay with fat burning exercises they become very easy. Coupled with the correct eating in a way that helps contribute to fat burning, this will result in a leaner, meaner you.

Every day, seven days a week, do some sort of aerobic activity. When your heart beats more quickly, not so quick that it struggles or your sucking in air hard, but sufficient to make you sweat you’re doing aerobic exercise. This includes walking, jogging, swimming, stationary bike, and a host of other activities. Include intervals into the aerobic work outs. These are bursts of higher activity that help boost the metabolism even more. That means sprint between periods of walking, or burst to speed while swimming at an easy pace.

The health guides that say to do aerobic exercise three times a week or even five times a week are wrong. The body needs this work every day. Pick an exercise routine you can maintain so that you can stick with it seven days a week for the rest of your life. In order to burn stored fat make sure to exercise before breakfast or at least before your next meal. Throw in an extra 10-30 minute walk after lunch or dinner to this exercise routine, in addition to the 30 minutes. Finally, look for opportunities to move. This can mean anything from parking further from the office so you have to walk from the car to the door, to taking the stairs instead of the elevator, to raking leaves rather than blowing them.

Tone and build muscles with weight training at least 3 during times each week. Bigger muscles make your metabolism speed into overdrive. A faster metabolism will use up more calories even when you’re not moving. You’ll also generally look and feel better.

Don′t skip meals. Doing so drops your body into starvation mode where it burns calories more slowly. A sluggish metabolism is no friend of fat burning. Another way to avoid slow metabolism is to make sure to eat enough every day. For most people this means eating at least 1200 calories each day. Any fewer and your body is back in starvation mode, working against your efforts to use up fat.

Fat burning workouts shared with healthy eating can create amazing results. It doesn′t have to be hard. In fact if it’s too hard you’ll lose motivation and quit the program. Too many people get hyped up on working out after Xmas, or just before the summer beach season, but don′t even make it for a month before the difficult workouts ground them into stopping. Don′t be one of the many short term health fanatics that will remain over weight until they can get it right.
