The human sensory system is designed to give us the information we need to manage the challenges of the world. We take in vast amounts of information every minute of the day, much of it not relevant, the brain deciding what is important and what is not. We are familiar with vision, hearing and touch and consciously and unconsciously use the incoming information to guide our actions and responses in daily life. However, there are two more sensory input systems, related to the others, which are vital to normal functioning. These are the sensory feedback we get from our bodies and the joint position sense.

Have you ever woken up in the night to find you have a numb and dead arm? I woke up on my back to find an arm laid across my chest so I lifted it off to the side. Very soon it came back. I moved it again, this time with a bit more speed. It came back. Gradually waking up I felt up the arm until I got to my shoulder. It was my own arm! Since I had laid on my arm, cutting off the blood supply to the nerve or compressing it, all sensory input to my brain from the arm had been cut off. My arm did not exist as far as by brain was concerned and when I gripped and moved my arm I had no sense that it was mine. As far as I was concerned the lack of feeling coming in meant that the arm had to be someone else′s.

By lying on the arm at night we can compress the arm nerves or the circulation to the arm and in this manner I had cut off all the incoming information from my arm to my brain. With this the brain concludes the body part does not exist so when we get hold of the arm there is no feedback and we don’t register the arm as ours. As I have worked as a physiotherapist for many years I have come across several incidences of the importance of sensory loss in managing normal movement. This has illustrated some important points about sensory input.

Once when I was treating a stroke patient he started to manipulate my hand back and forwards, straightening and bending the fingers and the wrist. I was puzzled at first until I figured out he thought my hand was his. His arm was completely numb and gave him no feedback so he couldn’t know that it wasn’t his arm by moving it as he never felt anything form it, until he followed it with his eyes and saw it was mine. If you get a dead arm try and do something with it, you will know how it feels unpleasant but also that it is very hard to get the arm to do anything despite your efforts.

Looking at a stroke patient it is easy to see the loss of movement but the abnormality of the sensory system is hidden and may make a higher contribution to the patient’s disability. Proprioception, also called joint position sense (JPS), is the ability of our brains to know where our joints are at every second. It informs us what the joint angle is, how much body weight is on it and how much muscular force is being exerted near the joint.

The brain is always monitoring the position of our joints so it can plan the movements we want to achieve. Without the position sense for our joints coming in normally to our brains we are unable to understand where our joints are and therefore we can’t predict what to do next. Without accurate joint position sense we cannot plan the next movement we need to do and so cannot move effectively.

Losing the sense of feeling our body parts accurately is fundamentally important to our ability to manage independent movement in our daily lives. Paraplegia, stroke and direct nerve trauma can cause loss of proprioception but lesser injuries can reduce this sense also. Anterior cruciate ligament rupture or even an ankle sprain can reduce the precision of the JPS and make treatment advisable. Physiotherapy rehabilitation skills are used to develop increased proprioception in many conditions and both stroke and sports therapists must be equally aware of its importance.

All the information which comes into our sensory system such as feelings like touch and pain is transmitted up to the sensory cortex of the brain, where the parts of the body have specific parts of the cortex dedicated to them. The diagram which results when the body parts are illustrated against the sensory cortex is called the homunculus, in which certain parts such as the lips and hands have much larger areas of brain devoted to them than average. This is because the importance for daily life of those areas is greater and more information is required from them to figure out what’s going on.

When we suffer an injury the pain comes directly from that part, streaming in from the highly irritated nerve ends and the normally silent nerves woken up by the chemical soup of the injury. As the barrage of impulses comes in to the spinal cord it meets the second stage nerves which will take the messages on into the central nervous system. These second stage nerves become highly excited by the incoming torrents of impulses and amplify the signal significantly, passing on much higher pain levels to the higher brain centres.

To feel any pain our higher brain areas such as our sensory cortex and our conscious interpreting minds must receive the pain impulses as pain is not present until it gets right through. No pain is imaginary but the brain builds a virtual feeling reality in order to interpret the world and be able to respond, a virtual reality involving sight, touch and pain. It’s vital to understand the concept that the brain is what makes the reality of the pain and not the injured, damaged or abnormal body part.

When a limb in amputated it is obvious the muscles, ligaments and bones are all cut, but what is less clear, and much more important for the future, is that the nerves travelling down the part are also cut through. Cutting the part of the nervous system off from the centre means a sudden loss of incoming signals from the amputated part, with serious side effects for the individual. When the nervous system is deprived of its incoming information the consequences can be unpleasant.

When normal incoming impulses are blocked from reaching the second stage spinal cord nerves these nerves overreact by becoming highly irritable and increasingly active. Due to the nerve transection and the complete loss of sensory input the second stage nerves start to go into business on their own, firing off pain signals without any incoming stimulation. As the sensory areas in the brain for the leg or other amputated part are still present, when they receive the spontaneous inputs from the overexcited second stage nerves they interpret them as pain in the area where the nerve used to come from originally.

Pain which appears in an area of the body which is now absent is known as phantom pain and is a common side effect of amputation which develops in the weeks and months after the trauma. Phantom pain can be very unpleasant in nature, very deep and cold, or sharp and stabbing and so can be a particularly difficult pain to treat or to cope with. Neuropathic pain is the term for a pain like this which is generated internally by the central nervous system and not as normal pains which are secondary to tissue damage.

Morphine related drugs such as codeine, tramadol, fentanyl and morphine may not be very effective against neuropathic pain so pain clinics more commonly prescribe drugs which affect nerve transmission of pain. These include gabapentin, pregabalin and amitriptyline, which are typically used for depression and epilepsy but have been found to have activity against pain. TENS (transcutaneous electrical nerve stimulation) can be useful, sending electrical stimulation in through electrodes on the skin. Coping with a long term pain problem can be addressed by cognitive therapy.

Because of the anesthesia, the child won’t feel a thing during the operation. And during the surgery, the tonsils are removed using an electric cautery, which is a burning tool, or a cutting tool. After the surgery, the child is given lots of fluids and after a day soft foods can also be given.
